Requirements

What medical conditions qualify for Medical Cannabis in Kentucky?

The following conditions would qualify you for your Kentucky medical cannabis card: any type or form of cancer; chronic severe, intractable, or debilitating pain; epilepsy or any other intractable seizure disorder; multiple sclerosis, muscle spasms, or spasticity; chronic nausea or cyclical vomiting; or post-traumatic stress disorder.


What types of Medical Cannabis products can I buy?

You can purchase and possess any type of medical marijuana product with a valid Kentucky medical cannabis card. This includes flower, edibles, tinctures, vape pens, and wax. However, smoking cannabis will be prohibited under Senate Bill 47. Gov. Andy Beshear and Kentucky lawmakers crafted this legislation to ensure patients can access the medicine they need while maintaining public health standards.

Kentucky Medical Cannabis Card Requirements

There are three basic requirements for the Kentucky medical cannabis card:

  1. You must be a resident of Kentucky.
  2. A state-licensed medical cannabis provider  must certify that you have one of the Kentucky medical cannabis conditions.
  3. You must not have a disqualifying  felon offense conviction.

A felony offense that resulted in the person being classified by the Department of

Corrections as a violent offender under KRS 439.3401; or

(b) A violation of a state or federal controlled substance law that was classified as a felony in

the jurisdiction where the person was convicted, except:

1. An offense for which the sentence, including any term of probation, incarceration, or

supervised release, was completed five (5) or more years earlier; or;

2. An offense that, as determined by the cabinet, consisted of conduct for which this chapter would likely have prevented a conviction, but the conduct either occurred prior to the

enactment of this chapter or was prosecuted by an authority other than the Commonwealth of Kentucky.


What documents do I need for a Kentucky Medical Cannabis Card?

The following information and documents are necessary when you register for the Kentucky medical cannabis card:

  1. Your ID and proof of Kentucky residency in the form of a state-issued ID or driver's license.
  2. A signed statement saying you will not share your medical cannabis (Notarized signature form).
  3. A signed certification from a medical cannabis provider licensed in Kentucky.
  4. Medical cannabis provider information.
  5. Designated caregiver information and attestation (if applicable).
  6. Notification of clinical trials.
  7. Attestation of information sharing.


Medical Documents

Patients need to provide some form of medical records to help the cannabis provider perform the evaluation. You should try to bring at least one of the following to your appointment:

  • Records or notes from a doctor's visit
  • Discharge summaries from a hospital visit
  • Referrals or notes from a chiropractor or other medical professional
  • Current prescription medication, or your prescription history printedfrom your local pharmacy
  • Proof of prior medical cannabis card status in another state

Cost

     Certification is $159 and is good for 1 year.
